Yes. This will always happen when you grab the Fire Keeper's Soul from Blighttown. I just had it happen to me on a brand new character that hadn't even set foot in the Undead Parish. Serves me right for using the master key I guess. For me it sucked because I was trying to see how far I could get at Soul Level 1 and had to fight Quelagg with regular Estus Flasks. Once the story has progressed a bit you will have the opportunity to use the Black Eye Orb you found and bring them back.

Nah, this has nothing to do with picking up the fire keepers soul from blighttown. I actually picked it up and gave it to her, lol.
But what you did was freeing the guy in the undead church. He then sits in front of that woman and just chills. After a while he will kill her and stop the fire.

I never saved the guy. I never even saw him. But the firekeeper is dead for me as well. Am I screwed?

It was an NPC that killed the lady under Firelink (Fire Keeper) and once you're in Anor Londo, that Black Eye Stone (I think thats the name) you got will start vibrating at a certain place (NOT avoidable) and you can then use it to invade that NPC's world and kill him to get back the Fire Keeper's Soul, and with it you can revive her.
